Accommodative hyperopes do not resist progressive lenses because they are difficult patients, they resist them because their visual system has been compensating for decades.

Why this topic matters

• Progressive addition lenses (PALs) remain the dominant presbyopic solution globally.

• Yet first-time progressive non-adaptation remains disproportionately high in accommodative hyperopes.

• The true issue is frequently under-recognised accommodative load and long-standing neural adaptation.

• Many of these failures are incorrectly attributed to:

o “Personality”

o “Lens design”

o “Unrealistic expectations”
