Activity Outline

Dr Ooi provides a comprehensive review of the assessment and management of patients presenting with fourth cranial nerve (trochlear nerve) palsy. The session will cover the relevant anatomical and physiological foundations, pathophysiological mechanisms, and clinical manifestations associated with this condition. Emphasis will be placed on systematic diagnostic evaluation, including key clinical tests, and on evidence-based management strategies. She will also  provide an in-depth review of visual field assessment in the context of Normal Tension Glaucoma (NTG). It explores characteristic patterns of visual field loss, their correlation with structural changes, and the clinical implications for diagnosis and management.

Dr Cassar will discuss the IOL technologies available and the expected outcomes. Deciding when to refer a patient for cataract surgery and preparing them for cataract surgery is dependent on numerous factors that will be considered in the lecture. An insight into how decisions are made to match the IOL to the patient’s expectations and ocular comorbidities. 

He will Also cover the changing landscape of intravitreal treatment for retinal disease. It will build upon current knowledge of anti-VEGF treatment and cover newer intravitreal treatments and their growing list of indications for use. The presentation also covers surgical interventions when intravitreal treatment is not indicated.

Learning Objectives

  • Perform the Park–Bielschowsky three-step test for the diagnosis of vertical muscle palsy.
  • Review the principles of ocular motility control, including Hering’s Law of Equal Innervation and Sherrington’s Law of Reciprocal Innervation, and their clinical implications.
  • Formulate an evidence-based management plan for patients with fourth nerve palsy, encompassing both conservative and surgical approaches.
  • Understand the anatomy and physiology of the visual pathway
  • Review the principles and interpretation of visual field testing in patients with Normal Tension Glaucoma.
  • Discuss evidence-based strategies for the management of Normal Tension Glaucoma
  • Differentiate benign from malignant eyelid tumors
  • Discuss treatment criteria for teprotumumab criteria
  • Learn when to refer for teprotumumab treatment of acute viral eye disease
  • Learn when to refer a patient for cataract surgery.
  • Learn what is the right and wrong thing to say to a patient about cataract surgery
  • Learn what can be reasonably expected with current IOL technology
  • Learn when to refer macular and retinal diseases that could be managed with intravitreal treatment or surgery.
  • Learn what has been recently released for macular disease and what is coming
  • Learn what to tell patients about expectations for their retinal conditions
