Obtaining a USI

If you do not already have a USI, (check your member portal to see if we have your USI on file). We suggest that you acquire your USI, following the steps below and send it to us at oqntevents@optometry.org.au. We will then store it in our secure database, ready for any training you are taking with us. You will continue to use your USI for any future nationally recognised training you undertake.

The Australian Government introduced the Unique Student Identifier (USI). From 1st January 2015, students undertaking nationally recognised training need to provide their USI to their registered training provider before they can receive their training records and result. As a Registered Training Organisation, our CPR Training Provider is obligated to collect this data prior to issuing a certificate.

Step 1: Go to http://usi.gov.au/Pages/default.aspx

Step 2: Select Create your USI from the front page

Step 3: Have any of the following available: * Driver’s Licence * Medicare Card * Australian Passport * Visa (with Non-Australian Passport) for international students * Birth Certificate (Australian) *please note a Birth Certificate extract is not sufficient * Certificate Of Registration By Descent * Citizenship Certificate * ImmiCard

Step 4: Provide the requested information as it is shown in your chosen form of identification.

Step 5: Save your USI as you will need this in the future.

Step 6: Please ensure you email our office at oqntevents@optometry.org.au your full name as it appears on your USI registration (for certification processes) and your USI number.
