OVC 2024: Optic nerve stroke - Blindness and worse (Recorded Webcast)
Activity Outline
While ischaemic optic neuropathy (ION) may result in very severe visual loss, its underlying causes can also have devastating systemic impacts. 95% of ION is non-arteritic (NA), and NAION is strongly associated with severe obstructive sleep apnoea (OSA). Untreated severe OSA in turn increases mortality four-fold, and systemic stroke and cancer three-fold. A presentation with NAION therefore mandates a sleep study. A second important aspect of NAION management is the use of oral steroid in the acute phase, which almost doubles the improvement in central acuity and peripheral field relative to the natural history. Both steroid and a sleep study are vital in the care of this blinding disease.
Learning Objectives
- Distinguish arteritic from non-arteritic ION
- List factors influencing normal retinal nerve fibre layer thickness
- Understand the link between NAION and obstructive sleep apnoea, and appreciate its implications for systemic health
